Bio

My passion and mission is to care for our most vulnerable, medically and socially underserved members of the community to improve health and quality of life in the primary care setting. As lead physician of the MESH Program, I disrupt a traditional primary care model by developing a complex care program for an at risk, high utilizing, Medicare population. My background in process engineering has enabled me to create sustainable operational processes for healthcare delivery to bridge silos creating an interconnected framework of health care professionals, reduce health system costs, and deliver equitable care. The multidisciplinary team intensively addresses social and economic determinants of health, and behavioral health. We review population health data to improve outcomes and reduce emergency department and hospital visits creating a cost savings for the hospital system.
